Plantar warts (verrucae pedis) are caused by the human papillomavirus (HPV), which infects the skin through tiny breaks or cuts. HPV has nearly 200 known variants, many of which infect the skin and can produce warts on the feet. These warts often develop on weightbearing areas and can become painful with walking or standing. What Are Plantar Warts?

Plantar warts are viral skin lesions caused by low‑risk HPV types that infect the outer layer of the skin. The virus thrives in warm, moist environments such as change rooms, pool decks, and public showers. Once inside the skin, HPV causes a thickened, spongy, bumpy lesion that may contain small black dots—tiny capillaries supplying the wart.

Most of the wart lies below the skin surface, often twice as large as what is visible.

Why Plantar Warts Develop

HPV enters through:

  • Small cuts or cracks in the skin

  • Walking barefoot in public areas

  • Moist, softened skin

  • Direct contact with infected surfaces

Although many people are exposed to HPV, not everyone develops warts. A strong immune response often clears the virus before a wart forms. People with reduced immunity due to stress, illness, medications, or systemic conditions, are more likely to develop persistent or multiple warts, because HPV clearance depends heavily on the body’s immune system .

Symptoms

Patients often notice:

  • A painful, well‑defined lesion on the bottom of the foot

  • A spongy or bumpy texture

  • Tiny black dots (capillaries)

  • Pain when walking or standing

  • A lesion that looks like a callus but hurts when squeezed

How Plantar Warts Are Diagnosed

Diagnosis is based on:

  • Visual appearance

  • Presence of capillary dots

  • Pain with side‑to‑side compression

  • Distinct borders

  • Skin lines that stop at the wart

This helps distinguish warts from calluses or corns.

Treatment Options

Over‑the‑Counter Treatments

Acid plasters, gels, and ointments may help destroy wart tissue.

Cryotherapy (Freezing)

Freezing and thawing cycles damage wart tissue and stimulate an immune response.

Prescription‑Strength Salicylic Acid

Higher‑strength acids help break down thickened skin and wart tissue.

Immune‑Based Approaches

Because the immune system is ultimately responsible for clearing HPV, treatments that stimulate local inflammation or support immune function can improve outcomes. Immunocompromised individuals often experience persistent, treatment‑resistant warts due to reduced viral clearance .

Combination Therapy

The case below shows excellent results using:

  • Cryotherapy
  • 40% salicylic acid
  • Vitamin A and zinc supplements
